Dogwood Animal Shelter - Volunteer Service Release
I agree to release, discharge, indemnify and hold Dogwood Animal Shelter Inc and it's members harmless for any and all damages to my personal property while performing as a volunteer, in a volunteer capacity, any and all duties for Dogwood Animal Shelter. I recognize that there exists a risk of injury or sickness, including personal harm in handling animals at Dogwood Animal Shelter in a volunteer capacity. On behalf of myself, my heirs, personal representatives and executor, I hereby release, discharge, indemnify and hold harmless Dogwood Animal Shelter, its agents, affiliates, servants, employees and Board of Directors from any and all claims, causes of action or demands, of any nature or cause connected to my volunteer services. This is to include any costs and attorney fees incurred by Dogwood Animal Shelter related to damage incurred or sustained by me in any way in connection with my volunteer services. Such injuries or damages might include, but are note limited to, animal bites, accidents, injuries, disease, personal property damage.
I further understand that public relations are an important part of volunteering at Dogwood Animal Shelter. I agree, therefore, on behalf of myself, heirs, personal representatives and executors to allow Dogwood Animal shelter to use my photograph or video recording taken of me for use in public relations efforts. Any photographic images or video tapes in which I appear will become property of Dogwood Animal Shelter, which can be used any and all ways it sees fit. Dogwood Animal Shelter will use reasonable efforts to notify me, but such pre-notification is not a pre-condition to the release of photograph under the auspices of Dogwood Animal Shelter.
I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTAND THE TERMS AND CONDITIONS OF THE FOREGOING VOLUNTEER AGREEMENT AND VOLUNTEER SERVICES RELEASE AND THAT i WILL COMPLY WITH THE SAME.
